Step by Step:
- Those interested in applying for CoverTN may complete the ONLINE VERIFICATION APPLICATION and return the signed and dated application to the State of Tennessee. The online application is only certified to work with Internet Explorer 5.5, Netscape 8 or higher browsers. Adobe Acrobat Reader is required to view and print your application once you have completed it. You must have your web browser set to allow for pop-ups. If you would prefer, you may call 1-866-CoverTN and request that an application be mailed to you.
- If you represent a small business applying for CoverTN for your employees, the business must qualify with the State first. Once approved, you will receive enrollment applications from BlueCross BlueShield of Tennessee(BCBST) for your employees to complete.
- Please have the following information available to you when completing the online application:

Federal Employer Identification Number (FEIN) – This is required for Business, Local County Governments and Self-Employed applicants only. The Federal Employer Identification Number is the nine (9) digit number (00-0000000) assigned by the Internal Revenue Service (IRS) to businesses that are required to file tax returns. If you do not have an FEIN, your Social Security Number (SSN) may be substituted. |

State Account Number – This is required for Business and Local County Government applicants only. The State Account Number is the eight (8) digit number (0000-000-0) assigned by the Department of Labor and Workforce Development that is used for the quarterly reports regarding all premium and benefit information (e.g. unemployment insurance) submitted to the Department. In general, if your business has an FEIN and employs 1 or more persons (other than the owner), then you should have a State Account Number. There are a few exceptions, which are listed in the help screens of the on-line application. Please check the exception that applies to your business. If these exceptions do not apply to your business, you must obtain a State Account Number from the Tennessee Department of Labor and Workforce Development (1-615-741-2486) before you can submit your Verification Application. |

Date of Birth – This is required for those applying as Self-Employed, Tennesseans Between Jobs and Employees at Non-Participating Employers. |

- Upon receipt and approval of your Verification Application, the State will notify applicants that they are qualified to participate in CoverTN and forward your information to BlueCross BlueShield of Tennessee (BCBST). BCBST will then send an enrollment kit with paperwork for you, and your employees if applicable, to complete. Return the completed paperwork to BCBST and soon your coverage will become effective.
